The FDA recently cleared the AmpliChip CYP450 Test (Roche), which analyzes blood-derived DNA to detect genetic variations in the activity of cytochrome P450 (CYP) enzymes CYP2D6 and CYP2C19 and determines the metabolizer status of the patient. The test is intended to help guide clinicians in prescribing individualized drug therapy. About 25% of all drugs, including many antidepressants and antipsychotics, are substrates of either CYP2D6 or CYP2C19. The test is being promoted initially to psychiatrists.

The FDA has approved over-the-counter (OTC) sale of the weight-loss drug orlistat (Xenical – Roche)1 as alli (GlaxoSmithKline), to be used in combination with a reduced-calorie, low-fat diet. Orlistat binds to gastric and pancreatic lipases, preventing absorption of about 30% of dietary fat with the 120-mg prescription dose, and 25% with the 60-mg OTC dose. Duloxetine (Cymbalta - Lilly) is the second drug to be approved by the FDA for treatment of fibromyalgia. Pregabalin (Lyrica), which is also approved for treatment of neuropathic pain and epilepsy, was the first. Duloxetine is a selective serotonin and norepinephrine reuptake inhibitor (SNRI) that is already marketed for treatment of depression and diabetic neuropathy.

antagonist mirikizumab-mrkz (Omvoh – Lilly) for
treatment of moderately to severely active ulcerative
colitis (UC) in adults. Mirikizumab is the first selective
IL-23 antagonist to be approved for this indication.
Ustekinumab (Stelara), an IL-12/23 antagonist, was
approved for treatment of UC in 2019. The IL-23
antagonist risankizumab (Skyrizi) is approved for
treatment of Crohn's disease.

Milnacipran (Savella - Forest), an oral selective serotonin and norepinephrine reuptake inhibitor (SNRI), is the third drug approved by the FDA for management of fibromyalgia. The 2 others are pregabalin, a gammaaminobutyric acid (GABA) analog also approved for treatment of diabetic neuropathy, post-herpetic neuralgia and epilepsy, and duloxetine, an SNRI also approved for treatment of depression, diabetic neuropathy and generalized anxiety disorder.

On January 10, 2013, the FDA issued a news release
to announce that it was requiring the manufacturers of
the zolpidem-containing products Ambien, Ambien
CR, Edluar and Zolpimist, which are all approved for
treatment of insomnia, to lower the doses for women
and to recommend in their labeling consideration of a
lower dose for men. No change will be required in the
dose of Intermezzo, a low-dose sublingual formulation
of zolpidem for middle-of-the-night awakening, which
is already recommended in a lower dose for women.
