The FDA has approved ixekizumab (Taltz – Lilly), an
injectable humanized interleukin (IL)-17A antagonist,
for treatment of adults with moderate to severe plaque
psoriasis who are candidates for systemic therapy
or phototherapy. Ixekizumab is the second IL-17A
antagonist to be approved for this indication in the US;
secukinumab (Cosentyx – Novartis) was the first.

The FDA has approved Odefsey (Gilead), a once-daily,
fixed-dose combination of the non-nucleoside
reverse transcriptase inhibitor (NNRTI) rilpivirine
and the nucleoside/nucleotide reverse transcriptase
inhibitors (NRTIs) emtricitabine and tenofovir
alafenamide, for initial treatment of HIV-1 infection in
patients with HIV-1 RNA (viral load) ≤100,000 copies/mL or to replace a stable antiretroviral regimen in
patients who have been virologically suppressed
(viral load <50 copies/mL) for at least six months with
no history of treatment failure.

Two recombinant vaccines, Arexvy (GSK) and
Abrysvo (Pfizer), have been approved by the FDA for
prevention of lower respiratory tract disease (LRTD)
caused by respiratory syncytial virus (RSV) in adults
≥60 years old. They are the first RSV vaccines to be
approved in the US. Abrysvo is also approved for
use in pregnant women at 32-36 weeks' gestation
to prevent LRTD caused by RSV in their infants from
birth through 6 months of age.

Transdermal fentanyl (Duragesic, and others) offers a convenient delivery system for patients with chronic pain1 but it has some drawbacks. The thionamides propylthiouracil and methimazole are both used to treat hyperthyroidism.1 Propylthiouracil causes severe hepatic toxicity or hepatic failure in about 0.1% of adults and children.2,3 It is the third leading cause of liver transplants due to drug toxicity (acetaminophen and isoniazid are the first two). Methimazole may cause less serious hepatic toxicity; reversible cholestatic jaundice has been reported. Some readers have asked why the June 2009 issue of Treatment Guidelines (Antimicrobial Prophylaxis for Surgery) did not recommend use of ertapenem (Invanz) for prevention of infection after elective colorectal surgery.
