The injectable glucose-dependent insulinotropic polypeptide (GIP)/glucagon-like peptide-1 (GLP-1) receptor agonist tirzepatide, which was approved by the FDA as Mounjaro for treatment of type 2 diabetes in 2022, has now been approved as Zepbound (Lilly) for chronic weight management in adults who have a BMI ≥30 kg/m2 or a BMI ≥27 kg/m2 and at least one weight-related comorbidity. The FDA has approved Wegovy HD, a 7.2-mg dose of the injectable glucagon-like peptide-1 (GLP-1) receptor agonist semaglutide, for use in adults who have tolerated the 2.4-mg dose for at least 4 weeks and require additional weight reduction.

The injectable glucagon-like peptide-1 (GLP-1) receptor agonist semaglutide (Wegovy), which was approved by the FDA in 2021 for chronic weight management in adults, has now been approved for chronic weight management in children ≥12 years old with an initial BMI in the ≥95th percentile for age and sex.Semaglutide is also available as Ozempic and Rybelsus for treatment of type 2 diabetes in adults.

The FDA has approved Kirsty (Biocon), a biosimilar to rapid-acting insulin aspart (NovoLog), for treatment of type 1 or type 2 diabetes. Kirsty is the first rapid-acting insulin to become available in the US that has received interchangeability status with NovoLog; Merilog, another insulin aspart biosimilar, was approved earlier but has not received interchangeability status with NovoLog.

The FDA has approved orforglipron (Foundayo – Lilly), an oral small-molecule, nonpeptide glucagon-like peptide-1 (GLP-1) receptor agonist, to reduce excess body weight and maintain weight reduction long term in adults with obesity, or with overweight and at least one weight-related comorbidity. Orforglipron is the second oral GLP-1 receptor agonist to be approved in the US for this indication; an oral formulation of the peptide semaglutide (Wegovy) was approved in 2025.

The injectable glucagon-like peptide-1 (GLP-1) receptor agonist semaglutide (Wegovy) has received accelerated approval from the FDA for treatment of noncirrhotic metabolic dysfunction-associated steatohepatitis (MASH; previously known as nonalcoholic steatohepatitis [NASH]) with moderate to advanced liver fibrosis in adults. It is the second drug and the first GLP-1 receptor agonist to be approved in the US for treatment of MASH; resmetirom (Rezdiffra), a thyroid hormone receptor-beta agonist, was granted accelerated approval for the same indication in 2024.

The sodium-glucose cotransporter 2 (SGLT2) inhibitor dapagliflozin (Farxiga – AstraZeneca) was approved by the FDA in 2020 to reduce the risk of cardiovascular death and hospitalization for heart failure (HF) in adults with heart failure with reduced ejection fraction (HFrEF). The indication has now been expanded to include a reduction in the risk of urgent HF visits and use in adults with any left ventricular ejection fraction (LVEF).

The FDA has requested the removal of the suicidal behavior and ideation warning from the labels of the 3 glucagon-like peptide-1 (GLP-1) receptor agonists approved for chronic weight management: liraglutide (Saxenda), semaglutide (Wegovy), and tirzepatide (Zepbound). The warning was initially included in the labels of these drugs based on an increased risk of suicidal behavior and ideation observed with older weight loss drugs.

It is the first drug to be approved in the US for this indication. Zepbound is also approved for chronic weight management in adults with obesity and in those who are overweight and have at least one weightrelated comorbidity. Tirzepatide is also available as Mounjaro for treatment of type 2 diabetes.
