The FDA has approved tirzepatide (Mounjaro –
Lilly), a peptide hormone with activity at both
glucose-dependent insulinotropic polypeptide
(GIP) and glucagon-like peptide-1 (GLP-1)
receptors, to improve glycemic control in adults
with type 2 diabetes. Tirzepatide, which is injected
subcutaneously once weekly, is the first dual GIP/GLP-1 receptor agonist to become available in the
US. Selective GIP receptor agonists are not available
in the US; GLP-1 receptor agonists have been
available for years.

primarily metabolized by CYP3A4 and 2C8 ...
The oral kinase inhibitors dabrafenib (Tafinlar – GSK)
and trametinib (Mekinist – Novartis) have received
accelerated approval by the FDA for use together
for a fifth indication: treatment of unresectable or
metastatic solid tumors with a BRAF V600E mutation
in patients ≥6 years old who have progressed
following prior treatment and have no satisfactory
alternative treatment options. The combination is not
approved for treatment of colorectal cancer because
of known intrinsic resistance to BRAF inhibition and
dabrafenib is not approved for use in patients with
wild-type BRAF...

of locally advanced or metastatic ROS1-positive
non-small cell lung cancer (NSCLC) in adults.
Repotrectinib is the third oral tyrosine kinase inhibitor
to be approved for this indication in the US; crizotinib
(Xalkori) and entrectinib (Rozlytrek) were approved
earlier.

The FDA has approved sugammadex (Bridion – Merck),
a selective relaxant binding agent, for reversal of
rocuronium- or vecuronium-induced neuromuscular
blockade in adult surgical patients. It is the first
selective relaxant binding agent to be approved in the
US. Sugammadex has been available in the European
Union, Japan, and elsewhere for several years. Previous
FDA reviews of sugammadex did not result in approval
because of concerns about a risk of anaphylaxis and
other hypersensitivity reactions with its use.

The FDA has approved Cinvanti (Heron), an IV
emulsion formulation of the substance P/neurokinin-1
(NK1) receptor antagonist aprepitant, for prevention
of acute and delayed chemotherapy-induced nausea
and vomiting (CINV) in adults. Aprepitant is also
available in oral capsule and suspension formulations
(Emend, and generics), and fosaprepitant, a prodrug of
aprepitant, is available in an IV formulation (Emend for
injection).
Zolmitriptan (Zomig) Nasal Spray for Migraine

Zolmitriptan (Zomig - AstraZeneca) nasal spray was recently approved by the FDA for treatment of migraine. It is the second selective serotonin receptor agonist ("triptan") to become available as a nasal spray; sumatriptan has been available in this form since 1997. Some patients with migraine cannot take tablets because of nausea and vomiting, and nasal sprays are more convenient than subcutaneous injections.
Drugs for Epilepsy
The Medical Letter on Drugs and Therapeutics • Jul 31, 2017 (Issue 1526)
such as simvastatin (Zocor, and
generics) and oral contraceptives. The INR should
be closely monitored in patients ...
Treatment of epilepsy should begin with a single
antiepileptic drug (AED), increasing its dosage
gradually until seizures are controlled or adverse
effects become intolerable. If seizures persist,
specialists generally recommend trying at least
one and sometimes a second alternative drug as
monotherapy before considering use of two drugs
concurrently. When used for the appropriate seizure
type, AEDs are roughly equivalent in efficacy. Drug
choice is usually based on factors such as ease of
use, adverse effects, drug interactions, presence of
comorbidities, and cost.
